CPS Security Student Camp 2019 - Participant Guidance

Please submit the following materials using the form below: 

Emergency Contact Form - In case of an emergency, we will use this form to contact each student's parent/guardian. This will also give us information regarding any medical conditions. This form requires a parent/guardian signature (if under 18).
Audio-Video Release Form - We will be taking pictures/videos of activities throughout the summer camp. This release form allows us to share these. Each student must complete this form.
Statement of Agreement Form - Vanderbilt's policy regarding Non-Vanderbilt Interns and Observers can be found here: https://www.vanderbilt.edu/provost/interns-observers.php. After reading the policy, each student will fill out their name on page 1 of this form. On page 2, they will again fill out their name, along with their signature and the date.
Assent and Consent to Participate Forms - As part of this camp, we are conducting research on learning computer programming with special emphasis on cybersecurity and robots. The Assent form is for the students to sign and the Consent form is for the parents to sign.
Letter from Guidance Counselor - A guidance counselor or principal's letter is required to confirm school enrollment. If a school has multiple student attendees, this letter can reference all enrolled students.
